ELIGIBILITY INFORMATION & SPORTS FORMS 2018-19

INSURANCE: For online registration for school/sport insurance, click HERE.
Chimacum High School students​ who participate in Cross Country, Swim & Dive and Wrestling will need to purchase a CHS ASB card, complete the PTHS sports paperwork and pay the sports fees through Port Townsend High School.

FEES: There is a $75 Athletic Fee, paid with the 1st sport.
Sport fees are: 1st sport $65, 2nd sport $60, 3rd sport $55, and an ASB card is $30. Checks payable to PT Schools #50.
FORMS: Sports forms will be available online on August 1, 2018.
PHYSICALS: PTHS Health Clinic, in the Stuart Building, is open for Sports Physicals 9 am – 3 pm on August 14, 16 & 21, 2018. Contact 360-385-9400 to schedule appointment. Physicals must be completed prior to starting sports.

COSTS: Sports participants must purchase an ASB Card ($30), pay an Athletic User Fee of $75, and a sports fee of $65 for the 1st sport, $60 for the second sport and $55 for the third sport played per year.
NOTE: ALL Student Athletes and Activity Participants must maintain full-time enrollment in FIVE classes, during their entire sport/activity season, to be eligible.
* If you do not attend PTHS, please complete this registration form, in addition to the other sports forms, and bring it to the ASB Office at sign-up time. (This form will also be available at the ASB Office.) *
